PLEASE NOTE: This form is to request a certificate of completion for the following courses/classes only: 30 Hour in Classroom Drivers Ed, 6+ Hours Behind-the-Wheel Driving Lessons, 6 Hour Defensive Driving Class. If you have recently completed the 30 hour online portion of your Drivers Ed course (eClass Joshua's Law) your certificate is currently being generated automatically and will be sent to you via email within 24-48 business hours. If it has been longer than 48 business hours since successful completion of the 30 hour Drivers Ed eClass, or if you need the certificate sooner than the standard delivery turnaround, please contact our office directly at (770) 685-1413 to inquire about rushed certificate options.
